To address post-discharge mortality in children, this project will implement the Smart Discharges program at several regional hospitals in Uganda. Admitted children will be screened using the innovative mHealth app. High-risk children will receive discharge kits and will be referred for post-discharge follow-up in their communities. A post-discharge surveillance program will ensure health-seeking occurs when needed, and statistics will be captured for evaluation and continuous model updating. The project expects a 25% reduction in post-discharge mortality, with the potential to save thousands of lives annually.
